Upon completion of the video, if you would like to schedule an appointment, please complete a short quiz (link available after video has been viewed). Once you have passed the quiz, you will be given a seminar completion form to fill out and fax to the UF Weight Loss Surgery Center prior to making an appointment. Please also print and complete the new patient forms and bring them to your first appointment.
